FAFSA CHECKLIST





                        EVERYTHING YOU NEED TO COMPLETE THE FREE APPLICATION FOR FEDERAL STUDENT AID


              Most colleges award aid on a first-come, first-serve basis. It is recommended that you complete the application
              as soon as it opens (October 1).You will need to complete the FAFSA for every year you are in college. Visit
              STUDENTAID.ED.GOV/SA/FAFSA to fill out your FAFSA.

               PERSONAL DOCUMENTS & INFORMATION

                     Student Social Security Number & date of birth

                     Parent/s Social Security Number/s

                     FSA ID for student and parent (if applicable)
                     FAFSA on the Web password


                     Driver’s license number and state of issuance

               PROOF OF INCOME & ASSETS

                     Taxable income from 2017 W-2 forms or other records of money earned or received

                     Untaxed income, including workers’ compensation, child support, housing, etc.

                     Bank statements and records of investments (if applicable)

               TAX DOCUMENTS

                     Copy of 2017 Federal Income Tax Return (IRS tax form 1040, 1040A or 1040EZ)

               COLLEGE & HOUSING INFORMATION

                     Names of colleges & universities you want to send the FAFSA to (up to ten total)

                     Know whether you will live on campus, off-campus or with family (this may affect your award amount)
